Please print up the following and give to your friends and family. For each one who brings one at their appointment, you will get a $25 VISA gift card. There no limits to how many referral dollars you may earn. If you would like us to mail referral cards with your name on them to your friends, please call Dr. Dulay's office and let the team members know what address to mail these to.
